外籍人员进境体检须知

【 受理对象】
申请来华定居，或任职、就业、学习在华居留1年或1年以上的外籍人员。

【预约情况】
请关注“海关总署国际旅行卫生保健中心”微信服务号后点击“业务办理”或网站预约（http://www.beijingithc.org.cn）。扫描二维码，预约成功后，请凭预约短信在约定的时间办理，逾期无效，请重新预约。

【证书领取】
1.领取时间：办理之日起第四个工作日后领取（仅限工作日下午
13:00-16:00）。如需办理加急，请向登记台工作人员说明情况。
2.领取方式：
（1）现场取证，持取证凭证领取。遗失取证凭证者须携带体检者本人身份证件原件及复印件。
（2）快递服务，可现场办理寄送业务，证书将于办理日四个工作日后送达。

【外国人境外体检验证须知】
1、持预约短信信息。 
2、身份证件：护照原件。
3、近六个月内拍摄的白底小两寸彩色证件照2张（4.5×3.5cm）（请预先剪裁好）。
4、验证材料：
 （1）《外国人体格检查记录》表所体现的内容应由中文或英文填写，其他语种不接受，签发日期应清晰明了,此表有效期从签发之日起六个月内有效。此表须包括受检者照片（加盖医院公章）、体检结论、医生签名、医院公章（同照片处公章一致）。
 （2）体检项目按《外国人体格检查记录》表的内容进行，并附上X光报告（或X光片）、心电图图纸或报告、实验室检测报告（至少包含HIV、梅毒检测）、超声检查报告等原始报告或者化验单。
 （3）体格检查及体检报告的签发必须由境外的公立医院完成；如在私立医院或诊所完成，体检报告应先在当地进行公正。
 （4）体检报告内容如须通过中国驻外国领（使）馆的认证的，申请人应向中心工作人员提供使馆的认证证明或者认证标签。
 （5）如无中国驻外国领（使）馆的认证证明或认证标签的，或提交的“体格检查记录表”内容有缺项漏项、无原始报告单或报告显示不清晰项的，现场医师会根据实际情况补检相应的项目以达到验证所需要求。提交的《外国人体格检查记录》表无效的须重新办理健康体检。

【Certificate Collection】
1、Collection time: after the fourth working day from the date of processing (13:00-16:00 pm on working days only). For expedited processing, please explain the situation to the registration desk staff.
2、Collection method:
(1)Collect on-site with a collection voucher. Those who have lost their collection vouchers must bring the original and a copy of the medical examiner's own identification document.
(2)Express delivery service, on-site delivery service is available, and the certificate will be delivered four working days after the processing date.

【Notice on Verification of Overseas Physical Examination for Foreign Guests】
1、Holding reservation SMS message.
2、Identity documents: original passport.
3、Two small two-inch color ID photos (4.5×3.5cm) with white background taken within the last six months (please pre-cut them).
4、Verification materials:
(1)The contents of the Physical Examination Record For Foreigners should be filled in Chinese or English, and the date of issuance should be clear. This form is valid for six months from the date of issuance. This form must include a photo of the examinee (stamped with the hospital seal), the medical examination conclusion, the doctor's signature, and the hospital seal (consistent with the seal in the photo).
(2)The physical examination items shall be carried out according to the contents of the Physical Examination Record For Foreigners, and the original reports or test sheets such as X-ray reports (or X-ray films), ECG drawings or reports, laboratory test reports (at least including HIV and syphilis tests), ultrasonic examination reports, etc. shall be attached.
(3)The medical examination and the issuance of the medical report must be completed by a local public hospital; if completed in a private hospital or clinic, the medical report should first be impartialized locally.
(4)If the content of the medical report has to be certified by the Chinese consulate (embassy) in the foreign country, the applicant should provide the staff of the center with the certificate of certification or certification label of the embassy.
(5)If there is no certification certificate or certification label from the Chinese consulate (embassy) in a foreign country, or if there are missing items in the “Physical Examination Record For Foreigners”, or if there is no original report card or the report shows unclear items, the on-site physician will conduct additional tests on the corresponding items according to the actual situation to meet the verification requirements. If the Physical Examination Record For Foreigners submitted is invalid, a new physical examination must be carried out.
